Privacy Policy
The Patient Safety Group LLC (“The Patient Safety Group”) believes that one of the most critical aspects to achieving the objectives of improved patient safety promoted on The Patient Safety Group website (the “PSG Site”) will be to assure our users that they know how their personal information, and responses to data requests, may be used or disclosed. Privacy is a serious matter in health care. Our goal is to encourage your participation in providing the pertinent information we need to facilitate our programs. We'll do this by respecting your privacy and making sure you know what will happen to the information you provide. The effectiveness of our programs depends on it.

This Privacy Policy describes how we collect information, what we do with it, and what control over the information you have. Information Collection and Use
The Patient Safety Group collects personal information when (a) you visit the PSG Site, (b) you register with The Patient Safety Group, and (c) you use The Patient Safety Group products or services.

When you visit the PSG Site, The Patient Safety Group automatically receives and records information on our server logs from your browser, including your IP address, The Patient Safety Group cookie information, and the page you request.

When you register with The Patient Safety Group, we ask for information such as your name, title, place of employment, email address, and other professional information. Once you register and sign in to our services, you are not anonymous to us. The Patient Safety Group collects information about your transactions with us, including responses to any questions and the date and time. Our Terms of Use specifically prohibit our registered users from posting any patient specific information on any of our websites. We reserve the right to remove any such content at any time without your consent.

When you use The Patient Safety Group products and services, we may charge a fee and will ask for your billing address and credit card number if we do so.

Information Sharing and Disclosure
The Patient Safety Group does not rent, sell, or share personal information about you with other people or nonaffiliated companies except to provide products or services you've requested, when we have your permission, or under the following circumstances:
  • We provide all our customers the opportunity to view reports that take into account the non-patient specific data entered by all our participants. These reports include charts and graphs, numerical comparisons and shared stories based on specific queries. We will not provide any personal information to users of the PSG Site, unless you permit that information to be published.
  • We provide the information to trusted partners who work on behalf of or with The Patient Safety Group under confidentiality agreements. These companies may use your personal information to help The Patient Safety Group communicate with you about offers from The Patient Safety Group.
  • We respond to subpoenas, court orders, or legal proceedings as is necessary and we exercise our legal rights or defend against legal claims as we need to.
  • We believe it is necessary to share information in order to investigate, prevent, or take action regarding illegal activities, suspected fraud, situations involving potential threats to the physical safety of any person, violations of The Patient Safety Group's Terms of Use, or as otherwise required by law.
  • We transfer information about you if The Patient Safety Group is acquired by or merged with another company. In this event, The Patient Safety Group will notify you before information about you is transferred and becomes subject to a different privacy policy.
Third Parties/Links
The PSG Site links to other web sites. Please recognize that we cannot control the use or security of your information once you have left our site. If you have any concerns regarding the security or privacy of our affiliates, please review their privacy policies or contact them directly. The Patient Safety Group is not responsible for the privacy practices, data collection, security or content of other websites.

The Patient Safety Group may set and access our cookies on your computer. A cookie is a small amount of data, which often includes an anonymous unique identifier that is sent to your browser from a web site's computers and stored on your computer's hard drive.

We use industry-standard SSL (Secure Socket Layer) encryption technology, a secure server, and other physical and procedural safeguards to protect the security of your personal information. Please understand, however, that no security system is impenetrable. We cannot guarantee the security of our databases, nor can we guarantee that the information you supply will not be intercepted while being transmitted to us over the Internet. Please keep your password private. We will never ask for it by phone or email, and neither should anyone else.

Viewing and Updating Your Information
Registered users may view and correct their personal information by accessing the "My Profile" link on their "My Account" page once they have logged into the website.

By using the PSG Site, you consent to the collection and use of your information as we have outlined in this Privacy Policy and our Terms of Use. If you do not agree to this Privacy Policy or the Terms of Use, you may not use the PSG Site. We may change our policies at any time and without prior notice. If we decide to change this Privacy Policy, we will post the changes to this page so that users may be aware of our actions. Your continued use of the PSG Site following a posting of changes to this Privacy Policy will mean that you accept those changes.
